Issue Definition:

The issue definition must include the;

• Outcome (measure that is affected), • Type of issue that is evident (e.g. unstable, multi-model, excessive variation, level of performance) and is to be investigated, and • Consequence/opportunity (the result of the issue, which may be good or bad). The issue definition must be based on what is proven, not opinion.
